Understanding Needs vs. Realities Through the Lens of SDOH

Training Length: 2 hours

Join us for Taking Action – Understanding the Interplay of HRSN and SDOH in Diabetes Care and Prevention Services, a learning event designed for CDCESs, Lifestyle Coaches, and anyone involved in program management, created and facilitated by DTTAC with the support of ADCES. Together, we’ll explore how health-related social needs (HRSN) and the broader social drivers of health (SDOH) impact both diabetes prevention and management. Learn practical strategies to identify and respond to these factors to support better outcomes for participants and patients.

This course will feature a self-paced module with a worksheet assignment (~30 minutes) to be completed before the live webinar to be held on Friday, August 15th, 2025 from 2:00 - 3:30pm Eastern Time.  If a registrant is unable to attend the live event, a webinar recording will be sent to all registrants. This course must be completed 3 weeks (21 days) after the live event to receive a certificate of completion.
